Job summary

Responsibilities

• Develop treatment plans and coordinate services with community resources.
• Refer and link Veterans to appropriate community and governmental resources.
• Provide consultation to new social workers and supervise staff as needed.
• Coordinate care with health care teams to meet patient needs.
