Job summary

New Ground, INC in Patchogue, NY is seeking a full-time Social Worker to assess, provide counseling, and connect clients to resources in a permanent supportive housing program for homeless individuals and families. Weekly LCSW supervision is provided, and the role emphasizes advocacy, case management, and collaboration with a multidisciplinary team.

The ideal candidate has an MSW and LMSW or plans to secure LMSW within 12 months, with experience in homelessness, trauma, or related areas.

Qualifications

• MSW degree required or pursuing LMSW within 12 months.
• Experience with homelessness, trauma, domestic violence and/or substance abuse is a plus.
• Strong interpersonal and communication abilities and time management.

Responsibilities

• Regular contact with clients including onsite home visits and meetings.
• Provide clinical assessments, counseling, and safety planning.
• Case management and coordination with service providers.
• Maintain documentation and participate in a multidisciplinary team.
