Benedictine Health System is a Catholic, mission-based, non-profit health system headquartered in Minnesota that provides independent living and long‑term care services for aging adults, including assisted living, skilled nursing and rehabilitation.

St. Gertrude’s is seeking a Minnesota‑licensed Social Worker to provide social service assessments, care planning, resident advocacy, and relationship building with patients in our Transitional Care Unit. Candidates must demonstrate strong, positive interpersonal skills, flexibility, and the ability to work in an environment with rapid changes.

This is an 8‑month temporary position.

• Current social work license
• One or more years’ experience in a skilled nursing facility
• Familiar with federal and state long‑term care regulations
• CPR certified
• Excellent verbal and written communication skills
• Able to read, write, and communicate in English
