Job summary

Villa Healthcare in Saint Louis Park, MN, a 100+ bed skilled nursing facility undergoing renovation, seeks a Social Worker to support residents’ social, emotional, and psychological needs and assist with the admission process.

You will participate in interdisciplinary care plan meetings, document progress notes, and help residents with alternative housing when needed. A Bachelor's in Social Work and MN LSW are required; healthcare experience is preferred.

Qualifications

• Bachelor's Degree in Social Work or related field.
• Licensed Social Worker in MN.
• Healthcare experience preferred.
• Must meet State and/or Federal requirements for the position.
• Basic understanding of computer technology.
• Ability to read, write and follow directions.

Responsibilities

• Complete psychosocial assessments and adhere to facility admission process.
• Provide for residents' social, emotional and psychological needs.
• Actively participate in interdisciplinary care plan meetings.
• Document progress notes related to residents' care plan.

Job Description

Position includes being responsible for the completion of psychosocial assessments, adhering to facility admission process and providing for each residents social, emotional and psychological needs. Actively participate in interdisciplinarycare planmeetings. Document progress notes that relate to residents care plan. Assist the resident with alternative housing as necessary.This is a full-time position.

Qualifications

• Bachelor's Degree in Social Work or related field.
