Job Summary and Qualifications

The Social Worker SW Case Manager PRN evaluates psychosocial needs, coordinates community services, and facilitates discharge planning for medical patients. You will act as a liaison between patients and community resources, ensuring smooth transitions across the continuum of care.

Your responsibilities include:

• Collaborating with healthcare teams to develop treatment goals and intervention plans addressing psychosocial concerns.
• Providing crisis and supportive counseling, and counseling on advance directives.
• Documenting assessments, treatment goals, interventions, outcomes, and discharge plans.
• Referring and arranging transfers to long-term care facilities as needed.
• Tracking and intervening to reduce Alternative Care Days (ACDs).
• Performing admission and concurrent reviews per the Utilization Management Plan.

Qualifications:

• Graduate of an accredited school of social work.
• Bachelor's in Social Work (MSW preferred).
• Licensed Clinical Social Worker preferred.
• 2+ years of social work experience in acute care preferred.
• Experience in case management or utilization review preferred.
