At Houston Methodist, the Social Worker II PRN (SW II PRN) position provides compassionate, clinical social work, psychosocial assessments, diagnosis and treatments, and complex discharge planning to patients and their families. Collaborating with physicians and the interprofessional healthcare team, this role emphasizes the social and emotional aspects of a patient's illness to facilitate quality care and positive outcomes. It utilizes case management skills to address social, financial, and psychological issues, ensuring psychosocial and continuing care issues are managed across the care continuum. The position also serves as a hospital-wide leader for psychosocial issues, discharge planning, and population disease management.

People Essential Functions
  • Model effective communication with healthcare team members and report patient and family data comprehensively. Use therapeutic communication to build relationships and support patients and families in clinical or ethical issues.
  • Provide staff education specific to patient populations and departmental processes. Serve as a preceptor and mentor, fostering professional growth.
Service Essential Functions
  • Lead comprehensive case management activities, assess high-risk patients, and participate in daily care coordination. Conduct full psychosocial assessments and address social determinants affecting health.
  • Manage psychosocial treatment plans, including crisis intervention, therapy, and group support. Assist with screening and managing victims of abuse, neglect, domestic violence, and mental health issues.
  • Establish educational goals with patients and families, providing resources and planning for post-discharge care. Educate team members on care goals and appropriate discharge levels.
Quality/Safety Essential Functions
  • Document assessments and modify care based on ongoing evaluations. Collaborate with team to improve safety and care transitions.
Finance Essential Functions
  • Utilize community resources effectively, coordinate referrals, and guide discharge planning to optimize outcomes and reduce length of stay.
Growth/Innovation Essential Functions
  • Educate staff on community resources and psychosocial impacts. Lead evidence-based practice projects and professional development initiatives.

Qualifications
  • Master’s Degree in Social Work (MSW) from an accredited university
  • Three (3) years of social services experience in healthcare
  • Licenses and Certifications: LCSW, LMSW (with additional credential), or equivalent as specified
Knowledge, Skills, and Abilities
  • Proficiency in English, effective communication, knowledge of community resources, and healthcare financial issues
  • Ability to work independently, exercise sound judgment, and utilize computer skills
Additional Requirements
  • Professional attire: Business professional
  • On-call availability and travel within and outside Houston area as required
