Job summary

Social Worker (Full Time)

How You’ll Contribute

• Interview patients, families, caregivers, and legal representatives to assess needs.
• Determine, prioritize, and arrange for internal and external services/interventions.
• Participate in case reviews to evaluate case management and progress.
• Consult with the healthcare team to promote, monitor, and evaluate compliance with treatment plans.
• Assist with discharge planning and processes, identifying appropriate resources (transportation, housing, healthcare, social/spiritual services) and providing referrals as part of the discharge plan.
• Supervise and/or train new staff, students, and interns.

What We’re Looking For
