Job Description

The Social Worker is a member of the core interdisciplinary team, primarily responsible for assessing psychosocial symptoms of patients and families, developing outcomes to alleviate these symptoms, and providing psychoeducation, supportive counseling, and casework interventions. The role also includes providing psychosocial education and consultation to the interdisciplinary team. Salary Range: $30-$40/hour.

Responsibilities include:

  1. Participating in the preparation, evaluation, and execution of the interdisciplinary care plan to ensure high-quality psychosocial care, continuity, and comprehensiveness.
  2. Performing bio-psycho-social-spiritual assessments of patients and families, not diagnostic assessments.
  3. Providing individual and family supportive counseling.
  4. Referring patients and families to community resources.
  5. Maintaining accurate and timely documentation in patient medical records.
  6. Attending weekly team meetings for care planning.
  7. Participating in patient case reviews.
  8. Providing psychosocial education and consultation to the team.
  9. Participating in supervision meetings and training; may supervise social work interns under supervision.
  10. Coordinating and communicating volunteer interventions within the care plan.
  11. Participating in psychosocial spiritual on-call rotations.

Job Requirements

  • Ability to travel as required.
  • Knowledge of community resources or willingness to acquire this knowledge.
  • Understanding of psychosocial issues affecting terminally ill patients and families, including intervention strategies.
  • Ability to work effectively within an interdisciplinary team.
  • Excellent listening and communication skills.
  • Ability to advocate for patients and families within various settings.

Qualifications

  • Master's of Social Work from an accredited institution (or grandfathered in per Medicare Hospice regulations).
  • At least one year of social work experience in healthcare.
  • Experience with terminally ill patients and families is preferred.
  • State licensure or certification as required.
