JOB SUMMARY:
Serves as the social worker for the GUIDE (Guiding an Improved Dementia Experience) program to connect patients and caregivers living with Alzheimer’s disease and related dementia (ADRD) with necessary medical and social resources. Provides social work services, complex case management, and care coordination to patients and their families, collaborating with primary care providers, neurologists, and community partners. Assesses needs for community resources and barriers to healthcare, makes referrals, and works with the multidisciplinary team to develop comprehensive care plans to address ongoing health and mental health needs related to dementia.
License/Registration/Certification:
Licensed Clinical Social Worker (LCSW) in Illinois, with preferred accreditation as an Accredited Case Manager (ACM-SW).
