Job summary

Hopkins Center for Rehabilitation & Healthcare in New York is seeking a dedicated Full-Time Social Worker to join our Subacute Care Team. The role focuses on psychosocial assessments, patient-centered planning, and helping residents and families navigate discharge options.

Qualifications

• Bachelor's Degree in Social Work required; Master's Degree preferred.
• Minimum one year of healthcare, rehabilitation, or long-term care experience preferred.
• Experience with discharge planning and interdisciplinary team collaboration.
• Strong communication, organizational, and interpersonal skills.
• Proficiency with Electronic Medical Records (EMR).

Responsibilities

• Complete psychosocial assessments and social service documentation per state and federal regulations.
• Participate in interdisciplinary care planning and resident care plan meetings.
• Provide counseling, support, and education to residents and families.
• Coordinate discharge planning and community services.
• Assist residents and families with adjustment, coping, and transition needs.
• Maintain accurate documentation in the EMR.
