Job summary

Roland Park Place in Baltimore, MD is seeking a Social Worker for our Skilled Nursing Facility & Assisted Living, full-time, to support residents and families through transitions with compassion and advocacy.

The role focuses on psychosocial assessments, discharge planning, care coordination, and collaboration with an interdisciplinary team to ensure high-quality, person-centered care in a nationally recognized continuing care retirement community.

Qualifications

• Licensed Certified Social Worker (LCSW) or Licensed Bachelor Social Worker (LBSW) in Maryland.
• Minimum of two (2) years of healthcare experience (skilled nursing, rehabilitation or long-term care).
• Experience with discharge planning, care coordination, and interdisciplinary team collaboration.
• Knowledge of Medicare, MDS documentation, and long-term care regulations preferred.
• Excellent communication, conflict resolution, and organizational skills.
• Compassionate, resident-centered approach with the ability to build trusting relationships.

Responsibilities

• Conduct comprehensive psychosocial assessments for new and existing residents.
• Advocate for residents' emotional well-being, dignity, and rights.
• Coordinate interdisciplinary care plans and participate in resident care conferences.
• Provide counseling, support, and crisis intervention to residents and families.
• Assist with discharge planning and coordinating post-discharge services.
• Connect residents and families with community resources and support programs.
• Facilitate family meetings and help resolve concerns and conflicts.
• Collaborate with physicians, nursing staff, therapy, admissions, and community partners.
• Complete MDS documentation and maintain medical records in compliance with state and federal regulations.
• Coordinate admissions, complete admission paperwork, and liaise with hospital discharge planners.
• Participate in regulatory surveys and maintain HIPAA compliance.

What You'll Do

As our Skilled Nursing and Assisted Living Facility Social Worker, you'll play a vital role in supporting residents and families throughout their assisted living and healthcare journey by:

What We're Looking For

Required Qualifications:
