Luther Acres, part of Luthercare, seeks a Social Worker to support residents in skilled nursing at our Lititz, PA campus. The role focuses on admission support, care planning, and advocacy within a compassionate, team-driven environment.

The ideal candidate has a BS in social work, a PA driver’s license, and 3+ years of relevant experience, with opportunities for career growth and access to campus facilities.

Qualifications

• Bachelor’s degree in social work is required.
• Three (3) years of directly related social work experience.
• Knowledge of applicable state and federal regulations.
• Ability to interpret complex standards and write concise reports and histories.

Responsabilités

• Obtains social and medication history, financial information, and other information for admission. Explains policies and costs to residents, families, and physicians.
• Maintains contact with residents to assist with adjustment and attends progress meetings.
• Assists with discharge planning and applying for medical assistance.
• Participates in interdisciplinary resident care planning and committees; documents in medical records.
• Ensures compliance with healthcare standards and regulations; participates in safety and QA processes.
• Coordinates with Area Agency on Aging and Department of Human Services to provide maximum support; acts as resident advocate.
• Conducts follow-up after discharge to verify service delivery and address issues.
