Job summary

Blue Shield of CA is seeking a Care Management Social Worker to join the Care Management Clinical Social Work team in California. You will assess members, develop care plans, and deliver intensive psychosocial interventions to help address social determinants of health.

The role collaborates with Blue Shield nurses and providers, linking members to appropriate levels of care and community resources, and reporting to the Care Management Manager or Senior Manager.

Job description

Your Role

The Care Management Clinical Social Work team helps Blue Shield members with complex social determinants of health by conducting psychosocial assessments, developing care plans, and providing intensive interventions. The Care Management Social Worker will report to the Manager and/or Senior Manager of Care Management Clinical Social Work. In this role you will be assessing members and linking to appropriate levels of care, linking to community resources, and coordinating with BSC nurses as well as BSC providers.
