Job summary

Blue Shield of California is seeking a Care Management Social Worker to support members with complex social determinants of health. You will conduct psychosocial assessments, develop care plans, and coordinate with nurses and providers to link to appropriate levels of care and community resources.

Reporting to the Care Management Clinical Social Work Managers, you will assess, connect services, and deliver intensive interventions to improve member outcomes.

Job description

Your Role

The Care Management Clinical Social Work team helps Blue Shield members with complex social determinants of health by conducting psychosocial assessments, developing care plans, and providing intensive interventions. The Care Management Social Worker will report to the Manager and/or Senior Manager of Care Management Clinical Social Work. In this role you will be assessing members and linking to appropriate levels of care, linking to community resources, and coordinating with BSC nurses as well as BSC providers.
