Rochester Regional Health is seeking a Social Worker to support patients and families in their care and recovery. You will be involved in rounds, chart reviews, and multidisciplinary conferences to assess needs and advocate for insurance coverage.
You will coordinate referrals and develop care plans aimed at smooth discharge or continued care. Strong documentation, planning, and interpersonal communication are essential, with experience in a healthcare setting preferred.

SCHEDULE: Full Time
HOURS: 8am-4:30pm or 8:30am-5pm

Patient Care. Determine patient needs through rounds, chart review, patient/family interviews and team conferences; coordinate multidisciplinary and agency case conferences; work with care managers to advocate for patient/family to obtain approval for insurance coverageReferral Management. Manage referrals related to patients at risk and determine appropriate intervention strategies and document as implemented; ensure inappropriate referrals are channeled correctly and documentedTreatment Plan Development. Coordinate assessments and develop care plan in accordance with accepted social work policy; implement plan of intervention preparatory to discharge or initiate continued care plan

Sedentary - Sedentary roles are primarily office-based and require prolonged sitting (67-100% of the workday) with minimal physical effort and lifting limited to under 10 lbs. Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver's license.
$23.50 - $37.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
