The major duties and responsibilities include but are not limited:

  • Collaborates with facility and clinical leaders to determine priorities and clarify strategic direction and goals.
  • Offers staff, Veterans, families, caregivers' education, training, and assistance relevant to virtual care tools/technology
  • Ensures treatment plan goals, regarding technology and telehealth are being met and documented appropriately
  • Provides support to appropriate staff on a continuum of care as it relates to virtual care tool and program support for Veterans, including but not limited to integration, application, expansion, and direct customer service.
  • Acts as a coach to homeless program clinical teams integrating virtual health tools/technology into their workflow.
  • Acts as a coach to Veterans, families, and caregivers to integrate virtual health tools/technology into their life journey and helps identify and encourage Veterans to meet treatment plan goals related to technology
  • Collaborates closely with virtual health leads to ensure care coordination.
  • Uses standard data collection instruments to gather and analyze qualitative feedback from end users
  • Engages in outreach planning and activities to improve access to healthcare services via mobile/telehealth initiative
  • Assesses and documents their observations in Veteran's medical record
  • Provides a full range of social work services within commonly accepted standards of social work practice which Includes case work, psychosocial assessments, individual, family and group counseling treatment planning, discharge planning, address Advance Directives, and other services as appropriate
  • Responsible for addressing changing technology by providing ongoing training and education surrounding technology and devices and the evolution of case management to serve in a technological environment
  • Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies
  • Complies with criminal conflict of interest laws and Executive Branch Standards of conduct as to avoid conflicts of interest while serving as the Homeless-VHRC Social Worker
  • Actively participates in the Homeless-VHRC conference calls, other homeless program calls that support in technology development and implementation, and attends trainings offered by the Office of Connected Care and other community partners that are offering trainings as it relates to technology
  • Responsible for following local policy and SOP's for the Homeless-VHRC and all VA Homeless program policies both locally and nationally
  • Takes responsibility for improving professional skills, participating in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested
  • Documents all contacts with patients/families in the medical charts and/or LEAF system
  • Completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies
  • Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Veterans and has regular access to printed and electronic files containing sensitive data which must be protected under provision of the Privacy Act of 1974 and other applicable laws, Federal Regulations, VA statues and policy. Incumbent is responsible for protecting data from unauthorized release of access codes, etc., as stated in the computer access agreement which the incumbent signs
  • Assists in providing data regarding the program's social work services on a regular basis, participate in planning, organizing, and coordinating social work services in the program, and preparing and implementing the referral process of veterans to programs
  • Organizes community services on behalf of beneficiaries and develops working relationships and agreements with other organizations having responsibilities for them. Maintains a current and productive network of referral resources. Makes provision for referral services from other agencies, community resources and other Governmental agencies to resolve problems
  • Provides clinical supervision for unlicensed social workers and may serve as a field instructor to trainees.

Qualifications

Basic Requirements

  • Citizenship: Citizen of the United States.
  • Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
  • Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work.

GS-12 Social Worker (Program Coordinator) Experience: In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

In addition to the experience above, candidates must demonstrate all of the following KSAs:

  • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to provide training, orientation, and guidance within clinical practice.

Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS[1]12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.

Physical Requirements: Light lifting (under 15 pounds); light carrying (under 15 pounds); reaching above shoulder; use of fingers; both hands required; walking; standing; near vision correctable at 13" to 16"; hearing (aid permitted); emotional stability. Occasional travel may be required to various VA Greater Los Angeles Healthcare System sites and community in order to provide support and meet clinical and technology needs with the veterans, their families, and representatives of community health.
