The U.S. Department of Veterans Affairs seeks an Intake Coordinator for Health Care for Homeless Veterans (HCHV) and HUD-VASH programs in Albuquerque. You will join an interdisciplinary team delivering psychosocial services to Veterans who are homeless or at risk.
You will provide direct patient care within the Homeless Patient Aligned Care Team (H-PACT) and coordinate clinical and administrative services to improve access to VA housing, health, and supportive programs.
As the Health Care for Homeless Veterans (HCHV) & HUD-VASH Intake Coordinator, functions as part of the interdisciplinary team and provides psychosocial services primarily to Veterans who are experiencing, or at risk of homelessness and in need of specialized support. Provides a combination of direct patient care services to Veterans being served by the Homeless Patient Aligned Care Team (H-PACT), as well as clinical and administrative service coordination for Veterans accessing HCHV programs. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Clinical experience conducting assessments and coordinating complex care. Working with individuals experiencing homelessness.
