Job summary

University Hospitals is seeking a PRN Social Worker to support patients at the Lake Health Geriatric Behavioral Health Center in Concord, Ohio. You will collaborate with the interdisciplinary team to assess psychosocial and discharge planning needs and to develop patient- and family-centered care plans.

Responsibilities include psychosocial assessment and intervention across all ages, discharge planning, linking to community resources, and ensuring PHI protection and policy compliance.

Qualifications

• Bachelor's Degree in Social Work (Required).
• Master's Degree in Social Work (Preferred).
• Licensed Social Worker (LSW) (Required).
• Experience in hospital or health agency settings preferred.

• Develops and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.

• Performs other duties as assigned.
• Complies with all policies and standards, including PHI handling.

We currently offer an opportunity for a PRN, Social Worker to support patients in Concord.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.

• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Education

• Bachelor's Degree from an accredited school of Social Work (Required)
• Master's Degree from an accredited school of Social Work (Preferred)

Licenses and Certifications

• Licensed Social Worker (LSW) (Required)

Work Experience

• Hospital/Health Agency experience. (Preferred)

• Theoretical and clinical knowledge regarding health care delivery (Required proficiency)

Position Details/Preferred Qualifications

Previous work in psychiatry or mental health is strongly preferred. Position involves psychosocial assessments and discharge planning of newly admitted patients into this locked, Geriatric Psych center. These patients need to be assessed within 24 hours of admission. PRN role is needed to cover weekdays, weekends, and holidays.
