Job summary

The University Hospitals social work team is seeking a PRN Social Worker to support patients at the Lake Health Geriatric Behavioral Health Center in Concord. This role collaborates with the health care team to assess psychosocial and discharge planning needs and to formulate patient- and family-centered discharge plans.

You will provide psychosocial assessment/intervention across all ages, develop discharge plans, link patients to community resources, and work in a multidisciplinary team.

We currently offer an opportunity for a PRN Social Worker to support patients in Concord.

A Brief Overview

Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.

What You Will Do

• Provides psychosocial assessment / intervention to assist patients/families. Provides age specific care to patients/families in all age groups.
• Develops and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develops plans with attention to individual patient and family goals
• Provides linkage to community resources to facilitate optimal care
• Communicates/collaborates with the work team using a multidisciplinary approach
