Trinity Health Mid-Atlantic’s St. Mary Medical Center in Bucks County seeks a part-time Social Worker (PRN) to provide psychosocial support to patients and families and collaborate on discharge planning.
The role requires a Bachelor’s in Social Work; a Master’s is preferred; at least one year hospital experience; weekday and weekend availability to cover day shifts; 16 hours weekly or per diem.

The Social Worker provides direct service to patients and families and works with community agencies, physicians and hospital staff in order to provide psychosocial support and to facilitate discharge planning. Modifies approach based on age-specific needs of patient population to include child, adolescent, adult and older adult.
