We are currently looking for a Per Diem Inpatient Social Worker who will monitor patients’ mental and psychosocial needs, coordinate services, and collaborate with the multidisciplinary team to ensure optimal outcomes.
The role includes clinical assessment, discharge planning, and facilitating individual, family, and group therapy.

We are currently looking for a Per Diem Inpatient Social Worker The Social Worker is responsible for monitoring the patient's mental and psycho-social needs and to provide the services to meet these needs in order to attain or maintain the highest practical level of physical, mental, and psycho-social well-being. The Social Worker coordinates and manages services provided to each patient to establish a focused, individualized program geared towards specific goals unique to that patient. They act as a liaison for patient, family and staff, overseeing day to day operations of all care provided to ensure successful patient outcomes. You will be responsible for clinical assessment, treatment of designated patients on assigned unit, discharge planning, participation as a member of multidisciplinary treatment team. You will use an understanding of principles of growth to assess each patient’s age specific needs and provide age specific treatment and care. You will also be responsible for conducting individual, family and group therapy by encouraging open communication and verbalization of feelings in an attempt to increase self – awareness and identify and resolve problems.
