Job summary

Responsibilities

Job Summary And Qualifications

Social Worker, Outpatient

Full time (Monday - Friday, days), no nights, no on call, no weekends, no holidays

Seeking a Social Worker, Outpatient for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do.

What you will do in this role:
  • Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support
  • Research and advocate for appropriate public assistance resources for clients
  • Referral to community resources, assistance with socio-economic problems
  • Communicate with client’s care team
  • Provide crisis intervention (referral to DCF, provider, etc as needed)
  • Coordinate treatment planning with specialists and outpatient providers as needed Evaluate current patients for appropriate social work services
  • Judge relative costs/benefits of potential social services and health care actions and choose the most appropriate one.
What Qualifications you will need:

EDUCATION:

Bachelors in Social Work (required)

EXPERIENCE:

Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse

  • Minimum of 2 years prior experience

CERTIFICATION/LICENSE:

State licensure or certification commensurate with degree
