Job summary

HCA Healthcare is seeking a Licensed Social Worker to perform evaluations and services for patients across diverse age groups. The role emphasizes patient-centered care, documentation, and collaboration with the interdisciplinary team in a high-quality healthcare environment.

  • Education: Post Graduate degree.
  • Experience: Minimum of 3-5 years’ experience.
  • Certification/license: A Clinical Mental Health Counselor license or Independent Clinical Social Worker license is required.

Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays.

Seeking a Licensed Social Worker who performs evaluations and services to patients of various age groups. Collects data and renders a professional opinion about treatment and care.

What will you do in this role:
  • Performs vocational rehabilitation assessment in a timely manner; dictates interdisciplinary report to clinical record within the required time frames.
  • Initiates and discharge documents within the required timeframe.
  • Documents of therapy notes on the clinical record.
  • Formulates and reviews initial and comprehensive treatment plans within the required time frame.
  • Identifies and documents patient care needs.
  • Works closely with the interdisciplinary team to ensure appropriate patient care. Knows, understands, maintains, and monitors patients’ rights.
  • Provides patient/family emotional support and education.
  • Meets in-service requirements including mandatory education.
  • Perform other duties as required. Must read, understand, and adhere to all Physician Services policies and procedures.
  • Practice and adherence to the Code of Conduct and Mission and Value Statement.
What Qualifications you will need:
Education:
  • Post Graduate degree
EXPERIENCE:
  • Minimum of 3-5 years’ experience
CERTIFICATION/LICENSE: 3 ways to qualify:
  • A Clinical Mental Health Counselor license or Independent Clinical Social Worker license is required in accordance with state regulations.
