Job summary

Intermountain Health is seeking an Oncology Social Worker (LCSW) to provide psychosocial care for cancer patients and families at St Josephs Oncology Center, Denver. The role requires collaborating with interdisciplinary teams, coordinating community resources, and supporting advance care planning.

The ideal candidate will have MSW, an active LCSW license, and at least 1 year of supervised health care social work experience; oncology certification is a plus.

Qualifications

  • MSW is required.
  • Valid LCSW licensed in the state you are practicing in.
  • Oncology Certification is preferred.
  • One (1) year of supervised social work experience in a health care setting is preferred.

Responsibilities

  • Provides continuing care assessments and reassessments of cancer patients and their families to assist in development of their treatment and survivorship needs.
  • Participates in oncology interdisciplinary rounds and multidisciplinary care planning with medical and community teams.
  • Provides community resources for psychosocial, financial, and educational needs of patient and family; collaborates with outside agencies.
  • Provides education on advance care planning and advanced directives; serves as a resource in financial support, disability eligibility, workplace issues, transportation barriers, mental health, substance abuse, domestic violence, child and elder abuse, and housing resources.
  • Provides education on palliative and hospice care, and coordinates referrals when indicated.
  • Assists in the development, implementation and annual review of policies, procedures and protocols related to Oncology Social Work.
  • Promotes mission, vision, and values of Intermountain Health, and abides by service behavior standards.
  • Performs other duties as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provides continuing care assessments and reassessments of cancer patients and their families to assist in development of their treatment and survivorship needs. Identifies psychosocial needs and access to appropriate services and resources.
  • Participates in oncology interdisciplinary rounds and multidisciplinary care planning. Communicates and collaborates with medical, interdisciplinary team staff, patient, family, and community agencies to provide continuity of care.
  • Provides community resources for psychosocial, financial, and educational needs of patient and family. Collaborates with outside agencies. Participates in community activities, community-wide inservices and training to promote psycho-social health.
  • Provides education and information on advance care planning and advanced directives. Serves as a resource in the areas of financial support, disability eligibility, workplace issues, transportation barriers, mental health, substance abuse, domestic violence, child and elder abuse, and housing resources.
  • Provides education on palliative and hospice care, and coordinates referrals when indicated.
  • Assists in the development, implementation and annual review of policies, procedures and protocols related to Oncology Social Work.

  • Performs other duties as assigned.
EDUCATION

Master's Degree in Social Work is required.

CERTIFICATION, REGISTRATION, LICENSE

Valid LCSW licensed in the State you are practicing in.

Oncology Certification is preferred.

EXPERIENCE

One (1) year of supervised social work experience in a health care setting working directly with individuals is preferred.

KNOWLEDGE, SKILLS, AND ABILITIES
  • Regular attendance to perform work on site during regularly scheduled business hours or scheduled shifts is required

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. $39.16 - $60.42
