Job summary

Intermountain Health in Denver, CO seeks a licensed Clinical Social Worker (LCSW) to provide psychosocial care for cancer patients and families. The role requires an MSW with LCSW licensure, with preferred oncology certification and 1 year of health-care social work experience.

Responsibilities include multidisciplinary rounds, resource coordination, and education on advance care planning. Tuesday-Friday? Monday–Friday schedule, 8:00 AM–4:00 PM, on-site at St. Josephs Oncology Center.

Qualifications

  • Master’s Degree in Social Work is required.
  • Valid LCSW licensed in the state you are practicing in is required.
  • Oncology Certification is preferred.
  • One year of supervised social work experience in health care is preferred.

Responsibilities

  • Provide continuing care assessments for cancer patients and families.
  • Participate in oncology rounds and multidisciplinary planning.
  • Connect patients and families with psychosocial, financial, and educational resources.
  • Educate on advance care planning and directives; address housing, transportation, mental health, and abuse resources.
  • Coordinate palliative/hospice referrals when indicated.
  • Assist in developing and reviewing oncology social work policies and procedures.
  • Uphold Intermountain Health mission and values; follow service standards.
  • Perform other duties as assigned.

Job Description:Provides social services to patients in the care site and/or assigned unit.Position Details:Monday - Friday, 8:00 AM - 4:00 PMNo holidays or weekendsLCSW requiredSpanish speaking preferredESSENTIAL DUTIES AND RESPONSIBILITIES1.Provides continuing care assessments and reassessments of cancer patients and their families to assist in development of their treatment and survivorship needs. Identifies psychosocial needs and access to appropriate services and resources.2. Participates in oncology interdisciplinary rounds and multidisciplinary care planning. Communicates and collaborates with medical, interdisciplinary team staff, patient, family, and community agencies to provide continuity of care.3. Provides community resources for psychosocial, financial, and educational needs of patient and family. Collaborates with outside agencies. Participates in community activities, community-wide inservices and training to promote psycho-social health.4. Provides education and information on advance care planning and advanced directives. Serves as a resource in the areas of financial support, disability eligibility, worklace issues, transportation barriers, mental health, substance abuse, domestic violence, child and elder abuse, and housing resources.5. Provides education on palliative and hospice care, and coordinates referrals when indicated.6. Assists in the development, implementation and annual review of policies, procedures and protocols related to Oncology Social Work.7.
