Job summary

Saint Luke's Health System in Kansas City, MO seeks a PRN Social Worker to assess psychosocial needs and provide counseling to patients and families during illness and crisis. You will help with discharge planning and transition efforts, coordinating with the care team to meet patient needs promptly.

The role requires an MSW and a Missouri LMSW license; hospital social work experience is preferred. Expect a flexible schedule with a minimum of 3 shifts per month and potential for more shifts as

• Master's Degree in Social Work
• Hospital Social Work experience is preferred

Responsibilities

• Assess the psychological, social, emotional, cultural and/or financial needs affecting patient health and recovery.
• Provide professional social work services using supportive counseling and clinical knowledge in illness crisis intervention and post-discharge planning.
• Collaborate with health care team to develop transition plans and address needs promptly; participate in program planning and development.

Responsible and accountable for assessing the psychological social emotional cultural and or financial needs that impact patients health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Help ensure our vision to be, "The Best Place to Get Care. Shift: PRN, 7:30 a.m. - 4 p.m. Minimum of 3 shifts per month, more available.

Education Required

• Certified LMSW Missouri Social Work license
• Master's Degree in Social Work
• Hospital Social Work experience is preferred
