Job summary

Saint Luke's is seeking a PRN Social Worker to assess patients’ psychosocial, emotional, cultural, and financial needs impacting health and recovery. You will provide professional social work services using counseling and clinical knowledge, collaborating with the health care team to develop discharge plans. Shift: PRN, 7:30 a.m.

- 4 p.m., minimum of 3 shifts per month with more available. Licensure requirements include Certified LMSW and Missouri Social Work license, plus a Master’s in Social

Qualifications

• Certified LMSW is required to practice in Missouri.
• Missouri Social Work license is required for licensure eligibility.
• Master's Degree in Social Work is required for this role.
• Hospital Social Work experience is preferred to support care coordination.

Responsibilities

• Assess patients' psychosocial, emotional, cultural, and financial needs impacting health and recovery.
• Provide professional social work services with counseling and clinical knowledge.
• Collaborate with the healthcare team to develop transition plans for discharge.
• Participate in program planning to meet patient and community needs.

The Opportunity

Responsible and accountable for assessing the psychological social emotional cultural and or financial needs that impact patients health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community. Help ensure our vision to be, “The Best Place to Get Care. Shift: PRN, 7:30 a.m. - 4 p.m. Minimum of 3 shifts per month, more available.

Education Required

• Certified LMSW
• Missouri Social Work license
• Master's Degree in Social Work
• Hospital Social Work experience is preferred
