Job summary

Saint Luke's seeks a Part Time Social Worker to assess psychosocial, emotional, cultural, and financial needs impacting patient health and recovery. You will provide professional social work services, collaborate with the care team, and contribute to discharge planning and community referrals.

The role includes family therapy sessions, psychoeducational groups, and participation in daily treatment meetings to support patients and their families during illness crises and post-discharge planning.

Qualifications

• Licensed MSW with state licensure or eligible licensure.
• Master's Degree in Social Work or related field required.
• Experience in clinical social work and patient-family counseling is preferred.

Responsibilities

• Assess psychosocial, emotional, cultural, or financial needs affecting patient health and recovery.
• Provide professional social work services using counseling and clinical knowledge.

Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participates in program planning and development to meet the needs of customers within the health care system and community.

Completes family therapy sessions with patients and their families.

Participates in daily treatment team meetings.

Provides psychoeducational group therapy sessions to patients.

Assists with discharging planning, making referrals and assisting with transportation arrangements
