Job description

Scope of Work

To work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. Responsibilities include:

• Conduct psychosocial assessments with patients and/or families to determine psychosocial needs or the need for an alternate site of care post‑discharge and execute a care plan.
• Communicate with alternate sites of care and community agencies in an effective and timely manner to best address patient needs.
• Complete and document assessments and care plans in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, and transition of care needs.
• Provide consultation and resources to members of the healthcare team.
• Maintain relevant and current knowledge of community resources.
• Participate or assist in department, regional, or statewide stakeholder committees, projects, etc., as assigned or requested.
• Develop and maintain current knowledge of federal and state regulations as they pertain to the role.

Qualifications

• Required: Master’s Degree in Social Work.
• Preferred: Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning.
• Required upon hire: Master Social Work License – State of Michigan.
