Job summary

The Medical Social Worker/Social Worker Assistant plans for and monitors services, interventions ensuring provision of quality care, plays role of a liaison between participant, family, caregivers and professionals.

MINIMUM QUALIFICATIONS

• Master’s degree or Bachelor’s degree from accredited school in Social work, Psychology or human related services
• Bilingual English / Mandarin or Spanish
• Ability to work with individuals with physical or mental limitations
