This job assumes responsibility for bio-psychosocial assessments, patient education, evaluation, referral to appropriate community resources, and supportive and crisis interventions. It applies thorough knowledge and use of company and community-based services to coordinate and implement complex discharge plans; collaborates and communicates (verbally and through the maintenance of the medical record) with the multidisciplinary team to (re) evaluate and respond to patient and caregiver needs, including emotional, social, cultural, and spiritual needs related to adapting to the patient’s medical condition. Additionally, this job may practice clinical social work within the agency only under the supervision of an LCSW employed by the company.

Education

Required - Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE)

Work Experience

Required - None

Preferred - Two years experience in a medical setting

Certifications

Required - Certified Social Worker (CSW) as verified by the LABSWE OR Registered Social Worker (RSW) certificate.

Preferred - Licensed Master Social Worker (LMSW) as verified by the LABSWE or scheduled to pass LMSW exam within 6 months of employment.

Knowledge Skills and Abilities (KSAs)

• Must have computer skills and dexterity required for data entry and retrieval of required job information.
• Proficient with Windows applications, keyboard, and role-specific software packages.
• Effective verbal and written communication skills; ability to present information clearly and professionally to various individuals throughout the patient care process.
• Working knowledge of common disease entities, disorders, and medical terminology to understand patients' and families' needs.
• Knowledge of acute and chronic care medical settings, medical terminology, and the psychosocial and economic effects of illness or trauma.
• Knowledge of healthcare systems and public programs (Medicare, Medicaid, etc.).
• Strong interpersonal skills to interact effectively and courteously with patients, families, team members, and community agencies.

Job Duties

• Maintain patient records, document, and communicate critical information to the medical team.
• Maintain professional knowledge and certifications through continuing education.
• Utilize management resources effectively.
• Participate in on-call rotation.
• Adapt behavior to specific patient populations, respecting privacy and communication preferences.
