• 62-bed Inpatient Psychiatric Center
• Geriatric Psych Unit (ages 65 and older). (Patients in this unit require assistance with all aspects of daily living - Total Care)
• ECT (Electroconvulsive Therapy)
• Social Workers are expected to conduct group therapy, psychosocial assessments, and discharge planning.

• 62-bed Inpatient Psychiatric Center
• Adult Psych Unit (ages 18-64)
• Geriatric Psych Unit (ages 65 and older). (Patients in this unit require assistance with all aspects of daily living - Total Care)
• Intensive Outpatient Program
• ECT (Electroconvulsive Therapy)
• Social Workers are expected to conduct group therapy, psychosocial assessments, and discharge planning.

Responsible For

The Social Worker strives to promote patient and family wellness, improve care outcomes, facilitate access to hospital and community resources, and develop comprehensive transitional care plans for patients with complex psychosocial needs in Inpatient and Emergency Department (ED) environments.

• Assesses, evaluates, plans, and coordinates community services
• Advocates for services to meet the specific needs of patients and families
• Conducts high-risk assessments within required timelines
• Coordinates with Care Manager daily
• Performs psychosocial assessments
• Provides patients and families with community resources
• Coordinates transition to post-acute care services
• Collaborates with the care team on complex cases
• Organizes family meetings and team conferences
• Identifies high-risk patients based on criteria
• Coordinates reporting to legal agencies as needed
• Facilitates financial assistance as needed
• Documents quality variances and discharge barriers
• Provides post-discharge follow-up
• Participates in readmission management initiatives

Qualifications

Minimum Education: Master's degree from an accredited social work program required

Minimum Experience: None required

Licensure/Certification: None required by law

Additional Qualifications: Prefer at least two years of experience in an acute or post-acute setting. LMSW in Georgia preferred.
