Job Summary:

Provides social work assistance to patients and families related to social determinants of health.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

• Interview patients, assess priorities, and document case activity.
• Consult with multidisciplinary team members and community resources to support patient treatment.
• Assist patients/families in obtaining community assistance through referrals.
• Develop and maintain relationships with community health, welfare, and social agencies, seeking innovative support methods.
• Participate actively with team members to improve standard work processes.
• Maintain alertness and ensure safety at all times.
• Perform other duties as assigned.

Education

• Master's degree in Social Work

Physical Requirements

• Frequent reaching, sitting, standing, walking.
• Use of vision and depth perception for near and far distances.
• Use of hearing and speech for communication, including alarms and machinery.
• Keyboard use and data entry.
