Job Summary:

Provides social work assistance to patients and families, focusing on social determinants of health.

Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
  • Interview patients, assess priorities, and document case activity.
  • Consult with multidisciplinary team members and community resources to support patient treatment.
  • Assist patients/families in obtaining community assistance through referrals.
  • Develop and maintain relationships with community health and social agencies, seeking creative solutions to meet patient needs.
  • Participate actively with team members to improve standard work processes.
  • Maintain alertness and work safely at all times.
  • Perform other duties as assigned.
Education
  • Master's degree in Social Work
Experience
  • No experience required
Physical Requirements
  • Frequent reaching, sitting, standing, and walking.
  • Use of vision and depth perception for near and far distances.
  • Use of hearing and speech for communication and safety alerts.
  • Keyboard use and data entry.
  • Occasional bending, stooping, climbing, twisting, and gripping.
  • Occasional lifting/carrying objects up to 25 lbs.
