Job Description

Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs, promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial, and developmental aspects of care.

Qualifications
  1. Education: Master of Social Work from an accredited School of Social Work, minimum required.
  2. License and Certification: Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
  3. Years of experience: One-two years of equivalent experience, minimum required. Three-five years preferred.
  4. Required Skills and Knowledge: Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning; techniques in individual, family, and group therapy; awareness of community resources, public assistance systems, Medicare/Medicaid; accepted professional and ethical standards within the existing medical community and professional organizations.
Responsibilities
  1. Engagement and Exploration: Engage patients and/or their families into a helping process. Assess the psychosocial and mental health needs of patients and families to identify barriers to treatment, enhance health status, increase satisfaction, improve resource utilization, develop rapport, and adhere to departmental responsibilities.
  2. Goal Development and Implementation: Develop and implement a comprehensive plan of intervention, provide counseling and social work interventions for complex issues, advocate and mediate for patient care, provide family education, assist with Advance Directives, initiate community resource referrals, and facilitate discharge processes.
  3. Evaluation and Termination: Summarize care plans, provide linkage to resources, evaluate patient outcomes, identify service barriers, assess transportation needs, and facilitate referrals to disease-specific agencies.
  4. Education and Consultation: Provide psychosocial issue training to healthcare professionals and collaborate on department development.
  5. Competency: Participate in customer service initiatives, attend training, maintain documentation, respond to requests, and engage in quality and safety initiatives.
