Position Overview:

Social Workers serving in various departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs, promotes continuity of care, and facilitates transitions to the next level of care. The role includes providing education, consultation, and acting as a liaison to the healthcare team regarding the emotional, psychological, social, financial, and developmental aspects of patient and family care.

Minimum Qualifications:

• Master of Social Work (MSW) from an accredited School of Social Work.
• Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
• For UCMC Psychiatric Services, NCI-APS Certification must be obtained within 90 days of hire.
• 1-2 years of relevant experience required; 3-5 years preferred.

Key Responsibilities:

• Engage patients and/or their families in the helping process, assessing psychosocial and mental health needs, identifying barriers to treatment, and enhancing health status.
• Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare teams.
• Provide counseling and social work interventions related to crisis management, mental illness, adjustment to illness, and other psychosocial barriers.
• Advocate, mediate, and negotiate to support patient health and social needs, including discharge planning and referrals.
• Educate families about care planning and assist with advance directives.
• Facilitate discharge and transition to appropriate care settings.
• Evaluate patient progress, summarize care plans, and respond to questions from patients and families.
• Provide education and consultation to healthcare staff on psychosocial issues.
• Participate in departmental activities, quality improvement, and community resource linkages.
