Amedisys is hiring a Social Worker MSW - LSW for Hospice in Maryland. This full-time, 1st shift role supports patients and families with social work services at home. The position emphasizes coordinating care, counseling, and ensuring timely communication among the care team.

Ideal candidates hold an CSWE-accredited MSW, have at least one year of healthcare social work experience, and possess CPR certification. Compensation reflects labor costs across markets and includes comprehensive benefits.

Qualifications

• Holds a master’s or doctorate degree in social work accredited by the Council on Social Work Education (CSWE).
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists the patient and family with health-related financial, social and emotional concerns and helps with coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures changes in clinical status are communicated to the health care team and care center staff promptly.
• Educates patient and family and other team members on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risks.
• Other duties as assigned.

• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.
