Job summary

Amedisys, Inc. in Kingsport, TN is seeking a Medical Social Worker for a full-time, Monday–Friday, 8 AM–5 PM role. An LMSW or LCSW is required to join the team delivering social work services in home health and hospice settings.

You will assist patients and families with health-related social concerns, coordinate care, and educate on treatment plans while following the plan of care. Salary ranges from $52,000 to $57,200 with comprehensive benefits.

Qualifications

• Master's or doctorate degree in social work from a CSWE-accredited program.
• 1 year of social work experience in a healthcare setting.
• Meets state and federal requirements for the position.
• Current CPR certification.

Responsibilities

• Assists patients and families with health-related financial, social, and emotional concerns and coping strategies.
• Provides social work services in accordance with the plan of care.
• Communicates changes in patient status to the healthcare team in a timely manner.
• Educates patient and family on social work interventions and treatment plan.
• Performs care planning, transfer, and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce them.
• Other duties as assigned.

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
