Job summary

Baylor Scott & White Health in Grapevine, TX is seeking a Social Worker MSW 1 to educate and advise patients and families on coping with emotional or physical issues. The role involves coordinating crisis intervention, bereavement, referrals, and discharge planning with the health care team.

You will develop and implement psychosocial assessments and collaborate on performance improvement initiatives to support patient-centered care and smooth transitions within the service line.

Qualifications

• Masters degree in social work with relevant licensure or eligibility.
• Experience in clinical social work or hospital social services.
• Strong assessment, communication and discharge planning skills.

Responsibilities

• Conduct and document psycho-social assessments to interpret patient needs.
• Develop, implement, and evaluate treatment and care plans with families.
• Coordinate crisis intervention, referrals, education, and discharge planning.
• Collaborate with the healthcare team to ensure positive patient outcomes.
• Prepare reports and maintain documentation for the care team.
