Job summary

Baylor Scott & White Health in Temple, TX is seeking a PRN/PT Social Worker MSW 1 to educate and advise patients and families on coping with emotional or physical issues, within the Texas State Board guidelines. Qualifications

• Master's degree in Social Work required.
• Licensed in Texas: LCSW or LMSW or LMSW-AP.
• 1 year of social work experience preferred.

Responsibilities

• Conducts detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
• Develops, implements, and evaluates treatment plans with patient education.
• Provides or supervises care plans including crisis intervention, prevention, education, advocacy, bereavement, therapy, and counseling.
• Liaises with community resources and staff for smooth transition and placement of patients.
• Collaborates with healthcare team to achieve positive patient outcomes.
• Participates in performance improvement initiatives including data collection and continuing education.
