Job summary

Baylor Scott & White Health in Fort Worth, TX is seeking a Part-Time Social Worker MSW 1 to educate patients and families, coordinate crisis intervention, bereavement support, referrals, discharge planning, and psychosocial assessments within the Case Management setting.

The role collaborates with the healthcare team to develop and implement performance improvement initiatives, ensure smooth transitions, and advocate for patients and families. A MSW and Texas licensure eligibility are required.

Qualifications

• Masters in Social Work is required.
• Must hold or be eligible for Texas LCSW/LMSW/LMSW-AP licensure.
• Experience coordinating psychosocial services is preferred.

Responsibilities

• Conduits and documents detailed psycho-social assessments to interpret social, environmental, and medical needs.
• Develops, implements, and evaluates treatment plans with patient/family education and involvement.
• Provides care plans, crisis intervention, prevention, education, patient advocacy and counseling with the healthcare team.
• Liaises with community resources and staff for smooth transitions and placement.
• Partners to deploy interdisciplinary care and achieve positive patient outcomes.
• Participates in performance improvement initiatives including data collection and continuing education.
• Prepares and writes professional reports, letters, and case summaries.

Job Summary

The Social Worker MSW 1 educates and advises patients and families on coping with emotional or physical issues. This is based on the Texas State Board of Social Worker Examiners' scope of practice. Coordinates services like crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning, and psychosocial assessments. Partners in developing and implementing performance improvement initiatives.

Essential Functions of the Role

• Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient.
• Employs advanced methods to develop, implement, and evaluate treatment plans. Provides patient and family education and serves as a professional resource.
• Provides or supervises care plans, including resource help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling. Partners with the healthcare team and involves the patient and family in developing and implementing plans.
• Serves as a liaison with community resources and staff. Ensures smooth transition and placement of patients within the assigned service line.
• Partners with other healthcare team members to deploy the interdisciplinary care of patients and achievement of positive outcomes.
• Participates in performance improvement initiatives to include data collection and review, development, and participation in continuing education programs.
• Prepares and writes professional reports, letters, and case summaries. Conducts and documents detailed psycho-social assessments. Interprets social, environmental, and medical needs of the patient.
