Position Summary: Position plans, organizes and implements social work services for patients and families within the Department of Pediatrics, Division of Endocrinology & Diabetes. Role will function in an outpatient setting, organizing and managing the social aspects of patient care for pediatric patients with complex care needs.

Job Description

Primary Duties & Responsibilities:

  • Works collaboratively with clinic staff to discuss patient care management.
  • Prepares psychosocial reports and maintains necessary records.
  • Assists with transportation needs, including for new onset diabetic patients who are transitioning from inpatient stay to first outpatient clinic visit.
  • Assists with scheduling patient appointments and provides telephone follow-up as needed.
  • Facilitates referrals and community resources for patients/families.
  • Assists with insurance coverage needs, including for new onset diabetic patients who are transitioning from inpatient stay to first outpatient clinic visit.
  • Answers inquiries from patients/families about community resources.
  • Assists patients/families with form completion for community resources.
  • Assists with completion of affidavits and hotline placements.
  • Other duties as assigned.

Salary Range: $56,200.00 - $87,100.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
