Job summary

Piedmont Augusta Hospital seeks a Social Worker to promote patient and family wellness, coordinate transitional care, and connect patients with hospital and community resources in inpatient and ED settings.

Responsibilities include developing comprehensive care plans for patients with complex psychosocial needs and scheduling for at least two days per week with potential third day; a Master’s in Social Work is required and LMSW licensure is preferred.

Qualifications

• Master's degree in Social Work is required.
• No professional experience is required, but candidates with relevant acute/post-acute settings are preferred.
• LMSW licensure is preferred or Georgia licensure eligible.

Schedule

• We are looking for someone who can commit at least two days a week (during business hours) and possibly a third from time to time.
