Job Summary:

Provides a wide range of clinical social work services in a primary care or home health setting, including support and consultation to care team in managing patients. Evaluates psychosocial assets and deficits to enable the patient to maintain the greatest level of independence. Coordinates with others to establish a care plan. Provides short-term counseling, teaching, and brokering with other resources for caregiving needs.

Essential Responsibilities

• Provide appropriate social work interventions to all age patients and their families, including psychosocial assessment, brief individual or family therapy, advocacy, linkage, referral, group work, case coordination, and long-term care planning and placement.
• Consult and coordinate with a multidisciplinary team of care professionals and primary care staff, including family and care conferences, team meetings, etc.
• Assess patient and family needs for care, communicate with healthcare providers, and collect relevant data throughout the care process. Evaluate psychosocial assets and deficits to support patient independence.
• Evaluate needs through timely assessment, communication, and observation. Modify care plans based on ongoing assessments.
• Provide crisis intervention, grief counseling, stress reduction, and development of coping strategies in various care settings. Conduct comprehensive psychosocial assessments, identify needs, and develop individualized care plans with patients and families. Discuss options proactively, including Kaiser and community resources. Collaborate with other disciplines, assist patient advocacy, and coordinate referrals and placements. Ensure effective communication among all parties involved. Perform other related duties as necessary.

Minimum Education:

Master's degree in Behavioral Science

Basic Qualifications:

• Minimum one (1) year of previous social work experience.
• Experience requirement waived for current KP Coalition employees per the National Agreement.
