Job Summary

Provides a wide range of clinical social work services in a primary care or home health setting, including support and consultation to the care team in managing patients. Evaluates psychosocial assets and deficits to enable patients to maintain independence. Coordinates care plans and provides short-term counseling, teaching, and resource brokering for caregiving needs.

Essential Responsibilities

1. Provide social work interventions including psychosocial assessments, advocacy, linkage, referrals, group work, case coordination, and long-term planning.
2. Collaborate with multidisciplinary care teams, participate in family and care conferences.
3. Assess patient and family needs, communicate effectively with healthcare providers, and evaluate psychosocial assets and deficits.
4. Conduct crisis intervention, grief counseling, stress reduction, and develop coping strategies.
5. Develop and implement individualized care plans, discuss options including Kaiser and community resources.
6. Coordinate referrals, manage scheduling, and evaluate appropriate care levels and placements.
7. Coordinate communication among Kaiser, families, and external care facilities.
8. Determine benefits application and facilitate communication across healthcare settings.

Minimum Education

• Master's degree in Behavioral Science

Basic Qualifications

• Minimum one year of social work experience (waived for current KP Coalition employees).

Additional Requirements

• Excellent communication skills, ability to work in a multidisciplinary team, knowledge of disease impact, and computer literacy.

Licenses and Certifications

• Licensed Advanced Social Worker, Licensed Independent Clinical Social Worker, Licensed Marriage and Family Therapist, or Mental Health Counselor in Washington.
• Basic Life Support certification required at hire.

Preferred Qualifications

• Two years post-master's experience, care coordination experience, knowledge of social case management, familiarity with electronic charting, and experience in medical or home health/hospice settings.
