Job summary

Qualifications

• Graduate from an accredited School of Social Work with an MSW degree.
• Current licensure as a Social Worker in New Jersey (LSW required).
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) depending on years of social work experience and clinical courses.
• Case Management Certification (ACMA or CCM) preferred.
• Minimum of one year of post-master’s in social work in the healthcare setting preferred.
• Strong writing and effective communication skills; ability to work as part of a multidisciplinary team.

MAJOR FUNCTION:Provides a variety of social service needs to the medical patient and caregivers as they transition to the appropriate level of care. Responsibilities include completing psychosocial assessments for specific patient populations including dialysis, mother baby and oncology, collaborating with the interdisciplinary team in discharge planning for high-risk patients. Participates in interdisciplinary rounds; educates patients and provides them as well as their caregivers with appropriate community resources and agencies to help ensure safety upon discharge.
