RWJBarnabas Health is seeking a Social Worker MSW to advocate for patients and families across the healthcare continuum. You will partner with physicians, nurses, and allied professionals to champion emotional and social aspects of care.

You will identify needs, coordinate discharge planning, provide counseling, and connect patients with community resources while documenting care in the EMR. Joining our Ambulatory Clinic team offers collaborative, mission-driven work.

Qualifications

• MSW degree from an accredited program.
• Active Social Work Licensure in NJ (LSW or LCSW).
• AHA BLS Provider Certification required.
• Meets accrediting/licensing standards (CSW, LSW, LCSW).
• One to three years experience in related field preferred.
• Ability to participate in weekend and holiday work rotation.

Responsibilities

• Identify, screen, and complete comprehensive clinical assessments for patients.
• Formulate timely discharge plans tailored to patient needs.
• Provide supportive counseling to patients and families.

As an essential member of our interdisciplinary teams, you will partner directly with physicians, nurses, and allied health professionals to ensure the emotional and social aspects of patient care are championed. As a Social Worker MSW, a typical day might include the following:

• Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations.
• Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient's unique needs.
• Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions.
• Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups.
• Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs.
• Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning.
• Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities.
• Entering accurate, clear, and timely clinical documentation and intervention records into the patient's electronic medical chart.
