Job summary

RWJBarnabas Health in Mountainside, NJ seeks a Social Work Assistant to support the patient social work department from admission through discharge, providing administrative help and coordinating care with licensed social workers.

You will assist with discharge planning, review of EMR data accuracy, and communication of regulatory updates to families. A strong understanding of the healthcare system and SDoH is important, with opportunities for involvement in department projects.

Qualifications

• High school diploma required.
• Associates or Bachelors degree in Social Work, Psychology, or a related human services field preferred.

Responsibilities

• Provide direct support to in-patient LTC and rehab social work departments.
• Assist SW in coordinating weekly team meetings for care coordination.
• Assist with ensuring regulatory communication notification to parents/guardians related to weekly meetings and facility/hospital notifications.
• Assist in mandatory notification to State and Federal regulatory entities; i.e. bed hold letters, room change letters, NJ State LTC Portal, OCCO portal submission etc, all tasks under direct supervision of primary social work and SW Director.
• Provide support to SW dept in regards to discharge planning and transition planning.
• Assist and support in review of patients EMR in ensuring accuracy of pts demographic information.
• Demonstrate understanding of health care system, managed care and resource utilization as well as policies and services available for patient populations.
• Effectively communicates financial and social needs barriers issues Social Drivers of Health, SDoH, to the primary SW department as appropriate.
• Participate in projects for the department that are consistent with department and hospital goals and missions.
• Assume additional department projects/tasks as needed.

Social Work Assistant

Summary: Assist in patient social work department in care coordination of patient, resident and family from admission through discharge. Provides the social work department with administrative support and assists with dissemination of regulatory communication and documentation, coordination and connecting residents and families to resources under the supervision of a licensed social worker

A typical day as a Social Work Assistant may look like:
