Job summary

Dartmouth-Hitchcock Health seeks a Social Worker to perform psychosocial assessments and counseling in the Emergency Department on Saturdays and Sundays from 8:00 AM to 5:00 PM, with three flexible days during the week. A sign-on bonus is eligible for this position.

The role involves coordinating community resources, supporting families through complex care, and documenting clinical data in the medical record. MSW is required, with preference for acute/ambulatory experience.

Qualifications

• MSW required.
• Experience in acute, home care, or ambulatory settings preferred.

Responsibilities

• Provides complete psychosocial assessments of patients and families.
• Provides counseling to individuals and families to help solve social and emotional problems.
• Coordinates community resources to address identified needs.
• Assists families in adapting to difficult dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
• Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
• Organizes support groups.
• Documents objective, concise and pertinent clinical data in the medical record.
• Perform other duties as assigned.

This position is Every Saturday/Sunday 8:00 AM - 5:00 PM in the ED and 3 flexible days a week
