Job summary

Agape Care Group is seeking a dedicated hospice social worker to evaluate psychosocial needs and provide ongoing counseling to patients and families facing life-limiting illness.

You will work within the plan of care to offer emotional support, coordinate with care teams, and contribute to a compassionate, supportive work environment with opportunities for advancement.

Qualifications

• Heart to serve patients and families; compassionate care.
• MS degree in social work from an accredited program.
• Current state license as a social worker.
• 2+ years of clinical experience in healthcare or hospice.
• Reliable transportation and ability to lift up to 25 lbs.

Responsibilities

• Perform psychosocial evaluations for patients and families.
• Provide ongoing counseling and emotional support per plan of care.
• Collaborate with leadership and participate in professional development opportunities.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
